Are the consent/intake forms required for services?
As a regulated health professional governed by the College of Nurses of Ontario (CNO), Nurse Erin is required to obtain informed consent and appropriate clinical information prior to providing any treatment. In accordance with CNO standards on Consent, Assessment, and Documentation, nurses must ensure that care is informed, safe, and based on accurate and relevant health information.
To provide the highest standard of care, patients (or their substitute decision-maker, when applicable) are required to supply essential medical details prior to the appointment. This includes identifying information such as date of birth, as well as relevant health information such as diagnoses, current medications, allergies, and other pertinent clinical history. Collecting this information allows for an accurate assessment, supports safe treatment planning, and helps identify any factors that may influence care.
If a patient has a legally appointed Power of Attorney (POA) for Personal Care or lacks capacity to provide consent, the POA or substitute decision-maker must review and sign the required consent forms on the patient’s behalf, consistent with CNO expectations regarding informed and substitute consent.
These requirements ensure that all care is delivered safely, ethically, and in full compliance with CNO professional obligations, while supporting the patient’s rights, well-being, and continuity of care.
